Upon arrival of your appointment you will need to provide our office with the following:

Dental and Medical Insurance cards or a claim form. We understand most dental plans do not provide a card, if that is the case you are responsible to obtain that information from either your dentist or human resource department of the employer. We will need to have the claims filing address, phone, group and identification number. Again, this information must be presented at the time of your visit. Please be prepared and have this handled ahead of time.

X-Ray and Referral from your referring doctor if your doctor is not forwarding that information to our office.
